Permanent Senior Practitioner - Community Mental Health Services

Head Medical is pleased to advertise an exciting opportunity for a Permanent Senior Practitioner in Community Mental Health Services on behalf of our client. This role offers the chance to make a significant impact within the community of Guernsey.
About the Role
The successful candidate will be an integral part of the Community Mental Health Services team, providing high-quality care and support to individuals experiencing mental health challenges. The role involves clinical leadership, assessment, and the implementation of effective mental health care plans. You will provide specialist support and advice to multidisciplinary teams in the management of service users presenting high risk, dual diagnosis or complex mental health conditions. The duty and brief intervention team receive referrals from GPs and a range of otehr healthcare professionals.
Key Responsibilities
- Provide comprehensive assessments for individuals with mental health conditions, whilst managing a full caseload and coordinate care for service users.
- Carry out comprehensive risk assessment and management plans for service users in a community setting.
- Liaise with other healthcare professionals including, Psychiatrists, GPs, Probation, Prison staff, Children's Services and Older adult services.
- Provide clinical supervision and training to junior staff and students.
- Ensure compliance with relevant legislation and organizational policies.
- Lead a team in emergency and acute situations, ensuring the patient's/client's needs are met and significant others are supported.
- To utilise information and data from a range of sources to identify patient/client problems and guide decision-making processes. Within field of expertise, and organisational policy, request specific clinical tests to establish a diagnosis
- To promote service user involvement in the planning, delivery and evaluation of care, respecting their wishes, beliefs and dignity.
Candidate Requirements
The ideal candidate should possess:
- A valid registration with NMC as a Mental Health Nurse.
- Demonstrable experience in a mental health setting, ideally in community services.
- Strong leadership skills and the ability to work collaboratively within a team.
- Excellent communication and interpersonal skills.
- A commitment to ongoing professional development and best practices in mental health care.
